of its capital stock stock has been subscribed, and paid.

Judicial Circuit, in and for Alachua

To the Honorable J.T. Wills, Judge of the Circuit Court for the
8th Judicial Circuit of Florida, in and for Alachua County:
Now on this, day came the undersigned incorporators and
respectfully represent unto your Honor that they desire to form
and organize, a Hospital Association, to be named "Gaines-
ville and Alachua County Hospital Association," to be located in
the City of Gainesville, in Alachua County, Florida.
And your Petitioners submit herewith the proposed Charter
of said Association subscribed by the intended incorporators,
setting forth the requisites of the Statute, and acknowledged
in accordance with law.
And your Petitioners further represent that the same has
been published in a newspaper in Alachua County Florida
for four weeks once each week setting forth briefly the
object and character of the corporation to be formed as
shown by the affidavit of the publisher, attached hereto
and made a part of this Petition.
Wherefore, your Petitioners pray that your Honor will
approve of this Charter, and will endorse your, approval
thereon, and the said corporation may become incorporated
in, accordance with the Statutes in such case made and
provided.
And your Petitioners will ever pray etc.
