E.E. Voyle who being duly sworn says that he is the
Treasurer of the Investment Company, a corporation or-
ganized and existing under the laws of the State of
Florida, and having its principal place of business in
the City of Gainesville, County of Alachua, State of Florida
and that ten per cent of its capital stock has
been subscribed and paid and this affiant has on
this date signed and sworn to, this affidavit in
duplicate

     The State of Florida
To All whom These Presents Shall Come = Greeting.
Whereas, J.H. Hodges, J.B. Padgett, W.R. Thomas,
R.D. Medlin, T.W. Shands, C.C. Pedrick, E.K. Fagan,
A.M. Cushman, Philip Miller, and T.F. Thomas on the
Twelfth day of October A.D.  1908. filed in the office of the
Secretary of State, a proposed charter of a corporation to
be known as National Odd Fellows Sanitarium Asso-
ciation with a capital of Ten Thousand ($10,000.00)
dollars for the purpose of carrying on, operating and con-
ducting a general sanitarium, to furnish board, lodging,
medicines, medicinal supplies, medical attention, or any
other matter or thing generally carried by or pertaining
to a general sanitarium establishment.  To establish phy-
siological, chemical and bacteriological laboratories for
medicinal investigation and research.  To conduct and
